Dates: There is no 2010 Lacrosse Camp at Columbia University. Please check back in the fall of 2010 for information regarding Summer 2011 camps.

Camp Medical Form:
A parent / guardian must complete the parent / guardian side of the medical form. A copy of the camper's latest physical (within a year of camp) can substitute for the physician side of the medical form.
